Road surface has large cracks/crevasse, pot holes and crumbling away

Specific details of the issue: Road surface on Woodglen Close is in very poor condition throughout. The photo attached is just one example, I have at least a dozen more. Damage includes potholes, large sections of asphalt crumbling away. Streets around us that were in much better condition were completely repaved last year but for some reason Woodglen Close was skipped.
